- Application Form with $150.00 application fee (non-refundable)*may be submitted prior to other documentation
- Application Essay
- Employment Resume
- Student Budget Worksheet (completed in full)
- Copies of High School Transcript and any educational degrees, diplomas, certificates, etc
- Scheduled (or completed) Application Interview *Zoom interview is an option if out of town
- Conditional Acceptance may be granted with all of the above items completed

- Three Reference Letters
- Health Assessment Form signed by both you and your physician
- Registered for (or completed) Introductory Workshop *must be completed before starting the program. Upcoming workshop dates: October 21-22, January 6-7. Registration form link below.
- Original & Certified High School Transcripts (must be forwarded directly to OVCMT from school)
- *CPR Required before end of Term 1. Standard 1st Aid with CPR level C (not occupational) *Must be from one of the following National providers: Cnd Red Cross, Cdn Ski Patrol, Heart and Stroke Fnd, Lifesaving Society or St. John Ambulance
- *Waived at this time - TB Test Required before end of Term 2. Phone to schedule – 1-888-288-8682
- *Meeting minimum entry requirements does not guarantee acceptance into the program. Preference will be given to applicants with recent Anatomy and Physiology or Human Biology.
- Note: Applicants without recent Biology or Human Science may need to complete the OV Human Science and College Preparation course (HSCP) prior to the course start date.
